(Well, not as simple as it might've been, but stylization seems, more or less, acceptable for quicker illustrations for lore.)

Aldram or Moon Harp is a specific kind of "warmonger tools", resonating with bearer's voice. Mostly these are forged with help of Calligraphy - but some are present in the world without touch of a living master smith.
This one here - is of such kind. Tentacle-like whips that this skull "makes" - are indeed somewhere, somehow, present. Be it this plain of existence or not - yet resonating with bearer's voice, it "pulls" these worms into the world, letting one who has this aldram, command it's tremendous power with ease.
A rare breed of natural "resonators"...
Alas, this one does not come for free - it seeps of bearer's health with time. Week teeth, scarred skin, cloudy mind, worsened vision... yet are these such a big price to pay for a weapon that can rip stones right from a castle siege wall?
First bearer was that one in the picture (nameless yes) - still having an idea-fix to find the skull again. Since then, it changed several bearers, was shape and smithed several times - and is now lost...
