I always wait until both slots are taken until I ask for payment and start the YCH. So far it hasn't ever taken more than a few days to get both slots filled for any of my YCHs, but I would say that after a week has passed and still no one has taken the second slot, I wouldn't mind if the person that already has a slot drops out at that point.
If you want both slots then you can bid on both slots, otherwise it goes to whoever else happens to win the auction for that slot.
