The red star denotes an air-to-air enemy kill. A very lucky plane (and pilot) indeed. From the looks of it, this is an F-4G "Wild Weasel", used for radar jamming. The A/A kill was either from short-range heat-seeking missiles mounted on the wing pylons, the medium-range missiles you see loaded against the fuselage, or a 30 mm gun pod mounted on the centerline.
How do I know such things? I was a 462 in the USAF for 22+ years, and the F-4E was my first plane. I love military aircraft...
