It says It's made in the fifth year of Chongning reign of Song dynasty in Bianjing(Now in Kaifeng, Henan) Official Kiln Ware by Wang's family.
It seems to be an ice-cracked porcelain (NOT means damaged). Ice-cracked porcelain has profound artistic value in Chinese artifacts due to its strict technology(Remained firmly cracked but not broken).
