This was a commission I recently purchased last week and it's actually a remake from an old sketch from few years ago that's lost forever.
Ever since Sakura became a student under her mentor and 5th Hokage, Tsunade, she managed to take in everything was thrown in her way. These include advance research in medicine/antidote, inherit both super strength and medical jutsu training from Tsunade. Sakura would also often taken whatever task/orders from Tsunade.

For why Sakura chose to get herself padded up could either be two opinions. One to follow in her mentor's footsteps and become as a strong kunoichi as her. The other reasoning is well an given direct order by Tsunade. Either way, Sakura gotten used to wearing diapers by now.

Speaking of orders, Tsunade is asking for a diaper change. Usually she'll asked for Shizune, Sakura, or even changed herself when neither are unavailable at times and so happens to be Sakura's turn this time.
