My polite side says it would be rude to ask for her number without knowing her, so I would introduce myself... doing my best to adjust line of sight towards her eye.
My shallow side says I would introduce myself and ask her name, because I want to see which side she speaks out of.
(Joke's on you, I work retail. She's pretty normal in comparison to some of the faces I see. If I can handle a man whose face is literally collapsing in upon itself, I don't think she would be too much of a shock ;P)
