Well, no one actually names the picture of the intersexed individual with big floppin' breasts "Herm With Big Tits!" They'd call it something like "<character name> Struts Hir Stuff". I could tell by the name and the thumbnail put together that this was just gonna be hilarious, so I clicked to see if it would indeed be hilarious. ...And it was.
