Look at the pages of hand drawings you've done. They maybe six months old, but they're still where you're at with your drawings.
So look at your own goddamned hands.
Put them in those positions.
Does it look like those drawings?
No.
No it does not look like those drawings.
Some are close, and that's good!
Some are very, very far off.
A big reason for this is that you don't structure your anatomy properly, and it throws everything off. Hands are not random shapes with lines hanging off them. There are bones, joints, and muscles.
From Andrew Loomis' Drawing the Head and Hands
Read these pages, study how Loomis structures his hands. I'm not tell you to draw like Loomis, so I don't want to hear any whining or indignation. These will show you how hands are built and how they work, and show you placement of muscles, forms and joints so that you can better structure hands when you draw them.

I was looking at my hands.
I also used this: http://www.posemaniacs.com/tools/handviewer/ (warning: the first few links on the right are NOT of hands)
