I know I'm two months late, but the pyramid helmet is supposed to represent suffering, if you've played the game, you can see that the monster itself is struggling with the helmet. the helmet, for lack of another way to decribe it, is a Iron Maiden you wear on your face, literally, the inside is covered with three inch metal spikes.
