What if the idea isn't what the commissioner expected when the sketch's done?

I mean, Not that they weren't happy with how you drew, but that they wouldn't be happy with the idea?

With sketches, I will give a basic skeleton before actually adding their character if they don't give me details. If someone is requesting a sketch, usually they should have a general idea of what they want, and if not it is basically artistic liberty.
