True. Didn't he also change his eye? I know he did have some of his intestines removed too. One thing that I thought strange; in his back story when he died he woke back up as a different person, yet in the game he dies loads of times and that never happened. Or am I just reading too much into it.
He woke back different, because he was killed by the shadows and by the will of Trecedent One. Thats why he lost all his memories. But when you killed by something else - you can't lose your memory, untill it won.t be cleaned by shadows.

Incidently, the guy who voiced the Transcendant One also did the voice for Kresselack the Black Wolf for Icewind Dale, and Xantam The Beholder/Ethon In Baldur's Gate - Dark Alliance, both of which are Forgotten Realms/D&D based games as well was Planescape Torment. Sadly he died in 2006, which is a shame since he had an incredible voice talent.
