Wells Fargo specifically lets you design your own if you like. Unfort there's a bar on one side of the card to signify that its personal. Some credit unions allow personal designs as well. You'd have to ask. I'm sure some let you make a personal one with a small fee. Wells Fargo had a range of designs, but they were too 'happy' and looked like a Hawaiian shirt.
There are some limitations. You can't use copyrighted material. To avoid any complications, I just didn't use any characters and made an abstract design.

I wish I could have had used my own design for a ebit card, but it requires to open an account online, the thing is you're done with the paperwork muc quicker by actually going there ^^

I guess it's easier where you live, paperwork here a ightmare, everything requires it, and we also happen to have very lazy administrative workers, so I tend to try and cut down the intermediates as much as I can.
And yeah it helps to see an employee to check if the forms are correctly filled in ^w^
