I have a few ideas for that...
1) Twilight was trying out a new Spell and Discord thought it would be fun to break her concentration. With the magical backlash (no idea if this is the right word) Twilight turns into a foal and Celestia punished Discord to be the Babysitter until a reversal spell is found.
2) Another form of Poison Joke. Discord found her and decide to take care of her to show he is now a good guy.
3) ... Discord did it.
