POV of the end of the car as it is stationed at the top of Lookout Mountain outside Chattanooga Tennessee. The track below descends at a grade of 72.5% making it the steepest passenger railway in the world.

The whole system is designed the same as an Otis Elevator safety system. Two cables are permanently attached to each car (one can easily carry the load) and the whole system is designed to completely lock down if the cars move faster than 10 MPH. The cars have air brakes which will lock in an overspeed event as well.
The only scenario that would result in fatalities in the failure of two multi-strand cables simultainiously that are only loaded to 10% of their tensil strength to begin with and having it occur at the top of the mountain.

They would average one wagon over the top every 2.5 minutes. The big winch at the top is a water brake to control the speed down the incline.
