1. Generally, people attach the foam pieces to the duct-tape dummy and then just tape up the DTD in the normal way to get the pattern. Again, there are tutorials and detailed instructions available on the Fursuit LJ.

2. I use a mixture of polyester stuffing and fur scraps - whatever I can get my hands on, really!

I've found that crumpled newspaper and paper bags work well with stuffing DTDs!
