He's from the film/book Shutter Island by Martin Scorsese. In the film, Leonardo DiCaprio is a cop trying to find the evil criminal Andrew Laeddis in a hospital prison for the criminally insane on a remote island. Not to spoil the ending, but the cop doesn't like what he finds in the end. Anyways...the film has a scene that looks a lot like this, inside the hospital's top-security ward that consists of an old fortress.
