Yes the "dentist" as it were, would have to be my favorite part of this sketch, not to say your other being isn't good, hes great too, i just have a thing for the more twisted and deranged, your use of blood works too, strong focus due to the fact its the only color on the image, but your use of highlights to me and your comments, does make me thing its more like jam or some sort of sweet gel. scratchiness of the drill conveys good motion though.
